Registering as a new patient with us

All new patients applying to join the practice will be asked to complete a simple application form (available from reception).

Please come to the GP surgery reception to collect a joining pack which includes:

  • ​New Patient Registration Information and Guidance

  • New Patient Registration Form – please complete for registration

  • New Patient Health Questionnaire – please complete for registration

Once we have received the Registration Form and Information Questionnaire the actual registration will be completed.  

The form will ask for your SMS preferences (whether you are happy to be contacted by the surgery through SMS).

You will be informed of your “Named GP” at this stage .

The new patient information questionnaire should be completed in full as this enables us to identify your health status and any special requirements you may have to help you access our services. If you require an interpreter, please let us know.

We are also taking on asylum seeker patients please feel free to enquire with our surgery.

We normally only accept patients in our catchment area. However, if you are not in our catchment area but would like still to register with us, please contact the surgery on 01922 624 605.

New Patient Health Check

New patients are invited and encourage to make an appointment for a health check within 1 month of joining the Practice.

The health check gives us the opportunity to update your health status and enables us to provide health services and lifestyle advice appropriate to your needs.
